Standards and challenges in the comprehensive therapy of CLP patients

Abstract

The treatment of CLP patients must permanently be comprehensive, multidisciplinary, starting from birth to adult age. From the intrauterine detection of the malformation, thanks to the early diagnosis through advanced imaging technologies, the counseling of parents during pregnancy focuses on comprehensive communication. This will facilitate compliance, understanding and improved preparation of therapy, as well as emotional support of the family.

The treatment planning takes place in optimal conditions, and includes, among other, dietary advice and feeding techniques.

Also, during this period, NAM (naso-alveolar molding) devices for modeling and conforming the lip, nose and palate are routinely applied in cases with indication. During the first months of life, the surgical interventions are scheduled, the first being lip surgery, followed shortly by the palate and alveolar process closure.

Depending on the growth, personalized orthodontic treatments, bone transplantation (osteoplasty), orthognathic treatment of deformities, evaluation of hearing, speech and language development, speech therapy both in childhood and at adult age, as well as aesthetic and functional nose surgery follow. After reaching the maturity of skeletal growth, bimaxillary orthognathic surgery and surgical interventions are indicated to improve the facial aesthetic appearance and also to improve breathing if necessary. In case of malformative syndromes and craniosynostosis associated with DLMP, craniofacial interventions are performed early, thus allowing favorable development.

Complex multidisciplinary treatment is the essential condition for a high-standard result for these conditions, which, if solved incorrectly or without following the protocol/stages, involve complicated procedures for the surgical repair of complications and sequelae.
